Throughout the History of Space travel starting in the 1930s until today, there have been a number of manned and unmanned rockets or space vehicles that have exploded. It is almost unavoidable when you are trying to learn to do something new (as in the aeroplane industry). The important thing is not to stop trying and to try to lean as much as possible about what happened to avoid it happening again and to make space travel safer and safer. We are still some ways away for routine access to space for humans but already some launchers world wide like Soyuz and Ariane have excellent safety records.
Simon is completely right. We have, very sadly, lost rockets from time to time. There are thousands of things that can go wrong, but there are many, many systems in place to try to catch things going wrong so that we don’t lose rockets. Sometimes it isn’t enough, and like Simon says, we try to learn from what went wrong.
